Helping Youth Stay Safe, Supported, and Connected at Home

Safe At Home West Virginia

When a child or family is facing complex challenges, the right support can help prevent disruption and create a more stable path forward. Safe at Home West Virginia brings families, professionals, and community partners together around an individualized plan that strengthens relationships, addresses immediate needs, and helps youth remain connected to their home communities.

Community-Based Support Centered on the Entire Family

About The Program

Safe at Home West Virginia is a state-funded program that provides temporary, intensive support for eligible youth and families experiencing significant challenges.

The program is designed to help prevent unnecessary out-of-home placements, support youth who can safely return to their communities, and reduce the need for children to be placed outside West Virginia.

Through Home Base Inc., participating families receive coordinated, family-driven support based on their individual strengths, needs, goals, and circumstances. Rather than relying on a one-size-fits-all solution, the Safe at Home approach builds a team around each family and creates a plan that reflects what will be most helpful to them.

These core goals and program requirements come from the West Virginia Bureau for Social Services.

How to Make a Safe at Home Referral

Accessing Services

Safe at Home referrals must be initiated by a West Virginia Bureau for Social Services worker. Families do not submit referrals directly to Home Base Inc.

If you believe a child or youth may qualify, speak with the family’s current BSS worker or contact a local West Virginia Department of Human Services office. Educators, healthcare providers, court personnel, probation agencies, law enforcement officers, and other community partners may also contact BSS regarding a potential referral.

Once an approved referral is assigned to Home Base Inc., our Wraparound team will connect with the family and begin coordinating the next steps.
