OUR APPROACH
What is Wraparound?
Support for the whole child and family. Wraparound is a strengths-based, family-driven process that coordinates care around the whole child. Instead of asking families to navigate multiple providers and services on their own, Wraparound brings everyone together around one individualized plan.
A Wraparound Facilitator works closely with the family to identify strengths, understand needs, establish goals, and build a team of professional and natural supports. Families remain at the center of the process, helping determine what support will be most meaningful and effective.
The goal is to create lasting stability by strengthening the family’s ability to overcome challenges, solve problems, and build connections within their community.

Finding the Right Support for Your Family
Our Programs
Home Base Inc. provides Wraparound Facilitation through two West Virginia programs. Although both use a family-centered and team-based approach, each serves children and families with different needs and eligibility requirements.
CSED Waiver Wraparound
CSED Waiver Wraparound supports children and youth with serious emotional or behavioral needs by providing intensive, individualized care in the home and community.
A Wraparound Facilitator brings the family, service providers, and natural supports together to develop a personalized plan of care. Services are coordinated to address emotional, behavioral, and everyday functional needs while helping the child thrive at home, succeed in school, and build lasting skills.
Safe at Home Wraparound
Safe at Home Wraparound helps eligible children and youth remain safely in their homes and communities. Families receive coordinated support to identify strengths, address challenges, strengthen relationships, and reduce the risk of out-of-home placement.
The program focuses on safety, stability, and equipping families with practical tools and sustainable support systems.
